We conduct quarterly staff evaluations (between employee and direct supervisor). This provides documented opportunities for performance feedback, communication, and development.

  1. All Midtown employees should complete a self-evaluation form and bring the completed form to the evaluation meeting with your supervisor.

FINANCE FORMS

IT SUPPORT

We work with BMISmartCloud to provide IT support services to our staff. They are available for remote and on-site support. Please see below to make your requests.

Helpdesk Request

Submit a helpdesk ticket, and they will respond during normal business hours: M-F, 7am - 4pm. This is for non-urgent matters (email, passwords, technical issues).

Public Calendar Events, Registrations, Facility Use

Reserve a room, request facility use, or create a public calendar event /registration.

Meetings or Classrooms

Planning a class, group, or meeting? Submit this form to request room usage. Please be prepared with details: dates, capacity, equipment needed, etc.

Event Coverage (Media)

Is your ministry having an event? You can request photography or video coverage from our Communications team, subject to availability.

Request Design/Printed Materials

Need more of a previously printed item or have a print-ready file? Submit requests for professionally printed materials such as signs, bulk flyers, or business cards. All requests are subject to Communications budget approval.

Request Website Update

Use this form for all website updates. Confirm details are final, match your ministry’s social media (if applicable), and your team is informed when submitting. This form keeps all requests organized, consistent, and in one place.

Microsoft Teams: Planner for Tasks (5 mins)

Adding tasks (a to-do list) in Microsoft Teams. You can use Planner in Teams channels for team tasks or for personal tasks. Learn how to add, complete, and manage tasks efficiently.

Microsoft Teams: "Bookings" for meetings (4 mins)

Planning Center (aka “PCO”)  is the family of apps we use in connection to our database. Download:

Midtown Photos

Smugmug is where we host our official Midtown Photos (taken by the photography team).  It’s password protected, and you may login to download photos for Midtown church marketing use. If you don’t have the current password, request from Marissa.
